Despite her still swollen face, she cautioned followers not to be alarmed and shared the challenges of her delicate post-operative phase, including a hospital infection and antibiotic replacement. This intervention reignited curiosity about her surgical journey, which has been transparently documented since the start of her transition. At the initial stage: the facial feminization package
In the early days of her transition, Maya underwent an intense set of procedures, some highly invasive, shaping the foundation of her current visual identity. These facial feminization surgeries, known as FFS in the medical field, altered the bone structure and tissues of her face. Common procedures included forehead reduction and reshaping, adjustments around the eyes and eyebrows, rhinoplasty, fat grafting to enhance cheekbones, and upper lip shortening. These surgeries occurred sequentially, part of a transformation she likened to ‘a rebirth’.
‘Sometimes, what we need is to go back to mom’s womb… but that option is not possible. I was surviving, not living. I had to take a break from everything to find myself. It was either that or depart for another realm. I needed to stay away from the internet and focus on my healing. But I’m coming back soon and I want to share everything with you all. You who have always been there for me,’ she posted on social media last year.

Breast silicone implantation
Another transition phase involved breast silicone implant surgery. Maya Massafera recently shared with her followers an appointment with a plastic surgeon. At 44, the influencer who transitioned gender last year expressed her intention to increase the silicone prosthesis in her buttocks to achieve the desired ‘big butt’. Currently at 300 ml, she aims to have 500 ml.
‘I want to swap my butt silicone for a larger one,’ she said on her ‘Maya’s Reality’ show on Instagram during the appointment.
Voice surgeries
Maya has openly stated she underwent two voice feminization surgeries. This delicate procedure, involving the vocal cords, necessitates a long, silent, and occasionally frustrating recovery. She admitted her voice initially sounded weak and unstable, gradually improving to her satisfaction months later. Following errors in the first operation, she underwent a second vocal cord procedure, informing her followers that she would need three months of voice rest.
‘I had two voice surgeries, with the most recent being 15 days ago. So, my voice isn’t ready yet; it will take another two and a half months, meaning three months after the surgery, to fully heal. But I’m here to show you the progress. I understand many of you are curious, given my years of media work on the internet and TV. I comprehend your curiosity,’ she said in a video.
Eye color change
The influencer traveled to France for an unusual aesthetic procedure: permanent eye color change. Opting for ceratopigmentation, a corneal pigmentation technique, she transformed her natural brown eyes into a blue-green shade. Costing $45,000, the procedure was detailed on ‘Maya’s Reality’ across her social media platforms.
‘I chose a green color, very similar to blue. Depending on the day, it appears green or blue. Remember, it’s like a tattoo: initially, it’s intense. So, in the first year, it will look very strong, eventually settling into the desired color,’ she explained.
Waist slimming
Having undergone over 20 aesthetic procedures, Maya openly shares her gender transition journey. In September 2025, she recalled one of the most challenging surgeries she endured: rib removal. ‘You’re wrapped in bandages for three months, like a tightly fitting corset you can’t remove except quickly for a shower. The bone needs to recalcify to shape the waist.’ She revealed losing approximately 10 centimeters of waist circumference post-process.
